Role Overview
In this position, you’ll be instrumental in guiding the Care Coordination, Referral Coordination, and Medical Assistant teams. Your responsibilities will encompass:
- Leading program planning and implementation across clinical operations, support, and population health.
- Monitoring performance metrics to uphold productivity and quality standards.
- Developing training initiatives to support staff within their professional scopes.
- Collaborating with internal and external partners to enhance patient care delivery.
- Analyzing data to identify trends and implement process improvements.
Qualifications and Requirements
Applicants should possess:
- A Bachelor’s degree in Nursing.
- Current RN license in good standing, with the ability to hold licensure across all 50 U.S. states.
- Minimum of 5 years in clinical settings and 5 years in people management roles.
- Proficiency in Microsoft Word, Excel, and Outlook.
- Strong organizational skills and the ability to prioritize in a fast-paced environment.
Preferred qualifications include:
- Certification in Care Coordination and Transition Management (CCCTM) or Certified Professional Patient Navigation (CPPN).
- Experience leading other people leaders.
- Excellent interpersonal and communication skills.
Compensation and Benefits
The role offers a competitive salary range of $95,000 – $105,000 annually, along with eligibility for performance bonuses. Benefits encompass:
